Explosion Protection | Operational Ex | Global Watch

Today's focus: Four fatalities during chemical-plant maintenance, a gas-cylinder facility fire, and the difference between equipment certification and operational safety.

Two serious incidents reported yesterday raise questions about pressure containment, hazardous substances and the management of non-routine activities.

For Heads of Operations, today's central question is:Who verifies that equipment is actually safe before somebody opens, connects, modifies or restarts it?

▶️ 1. INDIA - FOUR KILLED IN CHEMICAL FACTORY EXPLOSION

BREAKING INCIDENT | Gujarat - 15 October 2026

Four workers died yesterday following an explosion at Kovika Dyes & Intermediates in Chhatral GIDC, Gandhinagar district.

According to police, the workers were carrying out pipe-fitting activities near a vessel containing chemicals.

Preliminary findings suggest that a connecting pipe may have ruptured under pressure. Forensic investigators are examining the circumstances, including whether flammable substances were involved.

The explosion mechanism has not been established. It would be premature to classify the accident as an explosive-atmosphere ignition event. https://www.ndtv.com/india-news/4-killed-in-explosion-at-dye-factory-in-gandhinagar-12191201/amp/1

Why it matters operationally

Pressure containment and explosion protection are related but distinct safety disciplines.

Before maintenance on chemical-processing equipment, operators must establish:

Isolation → pressure release → draining → temperature control → chemical compatibility → atmospheric assessment → permit authorization.

A valid work permit cannot compensate for equipment that has not been made safe.

Head of Operations action: Review whether personnel are permitted to connect, disconnect or modify piping while process equipment remains pressurized.

▶️ 2. RUSSIA - FATAL FIRE AT GAS-CYLINDER FILLING FACILITY

CURRENT INCIDENT | Moscow region - 15 October 2026

A major fire occurred yesterday in an industrial hangar in Podolsk.

According to Russia's emergency authorities, one person died and three others were injured.

The fire affected approximately 2,400 m², and 23 people were evacuated.

Investigators reported that the facility was being used to fill domestic gas cylinders. Emergency authorities identified depressurization of a gas container in their preliminary account.

The exact sequence remains under investigation. https://www.interfax.ru/moscow/1122628

Why it matters operationally

Cylinder filling involves more than checking the condition of individual cylinders.

The complete operation includes gas transfer, pressure control, connections, leak detection, ventilation and emergency isolation.

Operational lesson: A cylinder may be compliant while the filling arrangement or operating procedure is unsafe.

▶️ 3. EU ATEX - OVERFILL PREVENTION DEVICES

REGULATORY WATCH | Existing 2026 decision

Commission Implementing Decision (EU) 2026/989, published on 5 May 2026, added EN 13616-1:2025 to the harmonized-standards framework supporting ATEX Directive 2014/34/EU.

The standard addresses overfill prevention devices incorporating closure mechanisms for static liquid-fuel tanks.

The reference to the previous 2016 edition is scheduled for withdrawal on 5 October 2027. https://eur-lex.europa.eu/eli/dec_impl/2026/989/oj/eng

Why it matters operationally

Overfill protection is often treated as instrumentation rather than part of the broader explosion-prevention strategy.

An overfill can create a flammable release even when every electrical device in the area is correctly certified.

Management action: Verify that tank overfill-prevention arrangements are correctly specified, tested and maintained, including the complete shutdown function where applicable.

▶️ 4. RESEARCH - HYDROGEN-REDUCED IRON DUST

2026 RESEARCH | Austria

Researchers at the Technical University of Leoben investigated the explosion characteristics of hydrogen direct-reduced iron (H-DRI) dust.

Their findings highlight two important issues.

First, the fine-particle fraction strongly influences explosibility.

Second, different ignition energies used during laboratory testing produced different explosibility classifications for some particle-size fractions.

Under the investigated conditions, the concentration of fine reactive particles was more informative than median particle diameter alone. https://onlinelibrary.wiley.com/doi/full/10.1155/amse/8840500

Why it matters operationally

As steel production increasingly incorporates hydrogen-based reduction technologies, material-handling systems may encounter unfamiliar dust hazards.

Engineering lesson: Do not assume that conventional iron-ore or steel-dust data adequately describe H-DRI dust.

▶️ 5. MANUFACTURER WATCH - Ex PRESSURE TRANSMITTERS

TECHNOLOGY REFERENCE | Existing product, July 2026 datasheet

Ashcroft provides the E2F explosion-protected pressure transducer for demanding industrial applications, including hydrogen, natural gas and petrochemical systems.

The manufacturer's documentation includes ATEX and IECEx conformity information, installation instructions and functional-safety documentation.

The E2F is offered with flameproof approvals, while related E2X configurations provide additional protection options. https://ashcroft.eu/product/e2x-and-e2f-explosion-proof-pressure-transducer/

Why it matters operationally

A pressure transmitter can simultaneously be an Ex device and part of a safety-related measurement function.

These are separate assessments.

Ex conformity does not establish that the complete pressure-protection function achieves its required safety integrity.

Verify the selected model, certification conditions, process compatibility, measurement performance and proof-testing requirements.

▶️ 6. COMPETENCE - INTRINSIC SAFETY WORKSHOP

CURRENT TRAINING | Germany - 15–16 October

DEKRA is conducting its EEX41 workshop on intrinsic safety, Ex i.

The program forms part of its 2026 explosion-protection training schedule. https://www.dekra.de/de/seminare-dtc-explosionsschutz/

Why it matters operationally

An intrinsically safe installation must be evaluated as a circuit or system, not merely as individual certified instruments.

Cable parameters, associated apparatus, device parameters and installation conditions all matter.

Competence lesson: Verify that personnel can assess the complete Ex i circuit before authorizing modifications.

▶️ 7. UPCOMING - EXPLOSION PROTECTION IN MUNICH

EVENT | Germany - 20–21 October

The Tag des Explosionsschutzes 2026, organised by TÜV SÜD, will address developments in explosion-protection regulations, standards and practical applications.

The conference brings together manufacturers, operators, inspection specialists and engineering professionals. https://www.de.endress.com/en/endress-hauser-group/events-training/summits/fachtagung-tag-des-explosionsschutzes

Why it matters operationally

Changes in standards must eventually be translated into engineering specifications, inspection procedures, competence requirements and operational decisions.

▶️ 8. OPERATIONAL Ex USE CASE - REPLACING A PRESSURE TRANSMITTER

PRACTICAL |

A chemical plant experiences repeated failures of a pressure transmitter.

Maintenance identifies a replacement with the same measurement range and process connection.

The new transmitter fits.

The control system receives the correct signal.

Production restarts.

But the replacement has a different Ex protection concept and electrical interface.

The original installation used an intrinsically safe circuit. The replacement requires different installation conditions.

What should have happened?

Engineering should have verified the complete certification and installation requirements before procurement and installation.

The change should also have been assessed for its impact on any safety-related function.

Operational lesson: A functionally equivalent instrument is not automatically an Ex-equivalent instrument.

▶️ TODAY'S Ex QUESTION

“Who verifies that a maintenance replacement preserves both the equipment's Ex integrity and the complete safety function?”

Consider pressure transmitters, gas detectors, valves, motors and control-system components.

For one recent replacement, establish:

Original specification → replacement specification → Ex assessment → installation verification → functional testing → documentation update → return-to-service approval.

If the evidence cannot be demonstrated, the maintenance process has a potential compliance gap.

DAILY Ex | CLOSING THOUGHT

Yesterday's fatal incidents remind us that industrial safety cannot be reduced to equipment certification.

A pressure vessel may be compliant but operated unsafely.

A gas cylinder may be certified but connected incorrectly.

An Ex transmitter may be suitable for its hazardous area but unsuitable for the required protective function.

The Head of Operations must ensure that equipment, processes, personnel and protective barriers work together under actual operating conditions.

Explosion protection is engineered. Operational Ex must keep it effective
