▶️ 1. IECEx - NEW REQUIREMENTS FOR CERTIFIED Ex REPAIR FACILITIES

🔴 NEW IECEx DOCUMENT | 13 October 2026

IECEx has published OD 314-5:2026, Edition 3.0, addressing quality-management requirements for certified facilities repairing, overhauling and reclaiming Ex equipment.

The revision aligns the document with IEC 60079-19:2025, Edition 5, and incorporates references to IEC 60079-0:2026.

Particularly important provisions concern repair records, personnel competence and equipment release.

The document specifies that actual measurements must be recorded; simply ticking a box to indicate a satisfactory result is insufficient.

Repair records must be retained for at least 10 years. Equipment release requires authorisation by a Responsible Person and appropriate documentation for the user.

The document states that it is to be implemented immediately upon publication.

Why it matters operationally

The operator needs evidence demonstrating what was repaired, what was measured, which acceptance criteria were applied and who authorised release.

Management action: Review the acceptance requirements for externally repaired Ex equipment.

▶️ 2. USA - CHEMICAL TANK EMERGENCY: 57 PEOPLE TREATED

🔴 INCIDENT UPDATE | Toledo, Ohio - 22 October

The emergency involving glycidyl methacrylate at MC Tank Terminal has developed significantly.

Authorities reported that 57 people had received hospital treatment, with seven remaining hospitalised on Thursday. None of those seven were reported to be in serious condition.

The evacuation zone was reduced to a quarter-mile radius.

The compromised tank continued releasing material, although its temperature and the release rate were decreasing.

Emergency authorities allowed the chemical reaction to continue because intervention could have increased the danger. The investigation into the initiating failure remains ongoing. https://www.wilx.com

Why it matters operationally

A chemical reaction may continue generating dangerous conditions after normal operations have stopped.

Operational lesson: Emergency procedures must address scenarios where immediate physical intervention is more dangerous than controlled isolation and monitoring.

▶️ 3. IECEx - NEW PROCEDURE FOR CERTIFICATION-SCOPE EXTENSIONS

🔴 NEW IECEx DOCUMENT | 13 October

IECEx has also published OD 212:2026, Edition 1.0.

It establishes procedures for extending the accepted scopes of IECEx Certification Bodies, Testing Laboratories and Additional Testing Facilities.

The process includes application review, technical assessment and decisions about whether additional site assessment is necessary.

New standard editions and technical changes are specifically considered when determining assessment requirements.

Why it matters operationally

A laboratory's IECEx acceptance does not mean it is accepted to perform every type of Ex test.

Procurement lesson: Verify that the certification body and testing laboratory have the relevant accepted scope for the equipment and standards involved.

▶️ 4. UK - JACKDAW OFFSHORE GAS FIELD APPROVED

🟢 CURRENT INDUSTRY DEVELOPMENT | 21 October

The UK government has granted consent for production from the Jackdaw gas field in the North Sea.

Gas supplies could begin before the end of October, following completion of development work.

The development involves offshore production infrastructure connected to existing facilities.

Why it matters operationally

Offshore gas installations require effective management of flammable releases, ignition sources, mechanical integrity and emergency shutdown systems.

Normally unattended installations introduce additional challenges involving remote monitoring, equipment availability and intervention time.

Management action: Verify that commissioning, pre-startup safety reviews and operational handover cover the complete installation—not only individual certified equipment.

▶️ 5. RESEARCH - GAS AND COAL-DUST EXPLOSIONS

🔬 RECENT RESEARCH | Published 15 September 2026

A study in Scientific Reports investigated how suspended dust affects gas explosions initiated by high-temperature ignition sources.

The research examined conditions relevant to methane and coal-dust mixtures.

The findings reinforce the importance of assessing interactions between combustible gases, dust particles and ignition conditions.

Why it matters operationally

Facilities containing both combustible gases and dusts may experience explosion behaviour that cannot be adequately characterised using either hazard alone.

Engineering lesson: Hybrid mixtures require appropriate material testing and scenario-specific explosion-protection assessments.

▶️ 6. HISTORICAL LESSON - EXPLOSION HIDDEN ABOVE A CEILING

📘 DOCUMENTED INCIDENT | USA - 29 January 2003

The U.S. Chemical Safety and Hazard Investigation Board investigated the explosion at West Pharmaceutical Services in North Carolina.

Six workers died.

Investigators established that fine polyethylene dust accumulated above a suspended ceiling.

Although visible production areas were regularly cleaned, combustible dust accumulated in concealed spaces.

The dust subsequently ignited, producing a devastating explosion.

Why it matters operationally

Housekeeping inspections must consider inaccessible surfaces, concealed voids, ventilation systems and other locations where combustible material can accumulate.

Field lesson: An apparently clean production area does not demonstrate effective combustible-dust control.

▶️ OPERATIONAL Ex USE CASE

The repaired Ex motor returns to the plant | PRACTICAL

A Zone 1 production pump motor develops a bearing defect.

The motor is removed and sent to an external repair facility.

The repairer replaces the bearings, checks the winding and performs mechanical and electrical tests.

The motor is returned with a completed service report.

Production urgently needs the pump.

Can Operations authorise installation and restart?

Not without verifying the relevant repair evidence.

The acceptance process must establish that the repair was performed against the appropriate technical documentation, that the applicable protection concept remains intact, and that required examinations and tests have been completed.

The operator must also verify the condition of the installation after reassembly.

The complete evidence chain:

Original certificate → repair specification → actual measurements → repair report → installation verification → return-to-service authorisation.

Owner: Maintenance Manager, supported by the Ex Responsible Person.

Measure: 100% of safety-critical Ex repairs accepted against documented technical requirements before return to operation.

▶️ TODAY'S Ex QUESTION

“When an Ex machine returns from repair, do we receive evidence of restored explosion protection - or merely confirmation that the machine works?”

A motor can pass an electrical functional test while having an unacceptable flamepath, incorrect bearing arrangement or unsuitable replacement component.

A successful functional test is important.

But it does not replace verification of Ex integrity.

DAILY Ex | CLOSING THOUGHT

Today's IECEx developments reinforce an essential principle.

Explosion protection must remain traceable throughout the equipment lifecycle.

The original manufacturer establishes the certified design.

The repair facility must preserve or appropriately address the relevant protection requirements.

Maintenance must verify installation.

Operations must authorise return to service.

And management must ensure that the evidence remains available.

Operational Ex is not simply about repairing equipment. It is about restoring and demonstrating the protection on which safe operation depends.
