Today's focus: Methane explosions, chemical-transfer errors and the hidden risks of changing material properties.

A fatal mining explosion in Türkiye, important new findings from the Toledo chemical emergency, and developments in inspection accreditation, Ex robotics and combustible-dust research lead today's edition.

Today's question for Heads of Operations: Are we managing the actual operating conditions—or relying on assessments that no longer represent the process?

🔴 1. TÜRKİYE - THREE KILLED IN COAL-MINE EXPLOSION

NEW INCIDENT | 23 October 2026

Three miners died and five were injured following a reported methane explosion at a privately operated coal mine in Kilimli, Zonguldak province.

Two injured workers were transferred to Ankara for further treatment.

Turkish authorities have opened an investigation. The precise ignition mechanism has not been established.

Why it matters operationally

Underground methane hazards require continuous control of gas accumulation, ventilation, ignition sources and operating conditions.

Gas monitoring alone is insufficient if alarms do not lead to appropriate operational responses.

Management action: Verify methane-detection coverage, ventilation effectiveness, alarm escalation and automatic equipment shutdown arrangements.

🔴 2. USA - TOLEDO CHEMICAL EMERGENCY: CAUSE CLARIFIED

NEW INVESTIGATION UPDATE | 23 October 2026

Authorities have identified an operating error during chemical transfer as the apparent cause of the Toledo tank emergency.

According to Mayor Wade Kapszukiewicz, an employee introduced a temperature condition during the transfer of glycidyl methacrylate that triggered a chemical reaction.

The tank ruptured and released material, forcing evacuations.

Approximately 60 people received hospital treatment, mostly for relatively minor symptoms.

All remaining evacuation orders were lifted on Friday.

Important clarification: Authorities confirmed that there was no fire or explosion. The incident involved a serious chemical reaction and tank rupture. The investigation continues. https://abcnews.com/amp/US/wireStory/toledo-ohio-mayor-employee-error-caused-chemical-leak-137517822

Why it matters operationally

A single operating deviation can initiate a dangerous reaction without an external ignition source.

Operational lesson: Temperature limits, chemical stability and transfer procedures must be treated as safety-critical operating conditions.

📘 3. ITALY - NEW INSPECTION ACCREDITATION TRANSITION

RECENT REGULATORY DEVELOPMENT | 20 October 2026

Italy's national accreditation body, Accredia, has published transition arrangements for ISO/IEC 17020:2026.

The new edition replaces ISO/IEC 17020:2012.

The international transition period ends on 31 March 2029.

In Italy, new accreditation applications have been accepted only against the 2026 edition since 1 October 2026.

Existing inspection bodies must prepare transition plans addressing management systems, resources, competence and independence requirements.

Why it matters operationally

ISO/IEC 17020 is relevant to inspection-body competence and impartiality, including organisations undertaking accredited industrial inspections.

It does not replace IEC 60079-17 or other technical Ex inspection requirements.

Management action: Verify the accreditation scope and transition arrangements of contracted inspection bodies.

🔬 4. RESEARCH - CAN A FEW DROPS OF SOLVENT CHANGE DUST EXPLOSIBILITY?

2026 RESEARCH | Published 15 July - Technical spotlight

Research published in Chemical Engineering Transactions examined the explosibility of powders containing small quantities of flammable liquids.

The researchers investigated magnesium stearate mixed with ethanol and wheat-derived material mixed with eucalyptus oil.

The experiments demonstrated that solvent addition can significantly influence ignition sensitivity and explosion severity.

The effects depend on composition, solvent concentration and particle behaviour.

Under some tested conditions, adding relatively small amounts of liquid increased explosion severity.

Why it matters operationally

This is particularly relevant to pharmaceutical granulation, food processing, powder mixing and chemical manufacturing.

Engineering lesson: Explosion characteristics measured for dry powders cannot automatically be applied to solvent-wetted mixtures.

⚙️ 5. TECHNOLOGY - HOW MANY ROBOTS ARE ACTUALLY IECEx-CERTIFIED?

TECHNOLOGY REFERENCE | August 2026 industry survey

https://ramtec.eu/ published an industry census identifying 65 IECEx certificates covering approximately 60 robotic models.

The survey also identified 57 certificates for robotic subsystems.

The figures reflect the company's August 2026 analysis and are not an official IECEx statistical publication.

Why it matters operationally

The growing use of robotic inspection, autonomous vehicles and mobile equipment in hazardous areas introduces important configuration-management challenges.

A certified robotic subsystem does not automatically establish conformity of the complete robot.

Procurement lesson: Verify the actual certificate, Ex marking, permitted operating conditions, accessories and complete machine configuration.

📘 6. IECEx / ATEX - VERIFY THE CERTIFICATE, NOT THE BROCHURE

GOOD ENGINEERING PRACTICE | Evergreen

The official https://www.iecex.com/ guidance emphasises that IECEx certificates must be verified against the online certification system.

The online record is the controlled reference. A printed certificate may have been superseded.

The EU's ATEX framework operates separately under Directive 2014/34/EU.

Why it matters operationally

A supplier's declaration that equipment is explosion-proof is not sufficient evidence of suitability.

Acceptance check: Certificate status → Ex marking → equipment configuration → special conditions → installation requirements.

📘 7. HISTORICAL LESSON - A NEW DUST COLLECTOR FAILED AFTER FOUR DAYS

DOCUMENTED INCIDENT | USA - 2012; investigation published 2015

The U.S. Chemical Safety and Hazard Investigation Board investigated a flash-fire incident at U.S. Ink in New Jersey.

Seven workers suffered burns.

A newly installed dust-collection system had operated for only four days before the accident.

Investigators identified inadequate airflow, accumulation of combustible dust and hydrocarbons, and insufficient commissioning.

The replacement system had been treated as an in-kind change despite significant design differences.

Why it matters operationally

New equipment is not necessarily safe equipment.

Historical lesson: Commissioning must demonstrate that the complete installed system performs as intended.

📅 8. NORWAY - ATEX CONFERENCE ADDRESSES OPERATIONAL CHANGES

UPCOMING EVENT | 18-19 November 2026

Standard Norge is organising its 2026 explosion-protection conference at Gardermoen.

The programme includes dust explosions, electrostatics, hazardous-area classification of older installations, hydrogen and ammonia ventilation, equipment responsibility and explosion isolation.

The event is supported by Norwegian regulatory and technical organisations.

Why it matters operationally

Older installations and modified processes require continued verification of their original explosion-protection assumptions.

▶️ OPERATIONAL Ex USE CASE

A small solvent addition changes the powder process

PRACTICAL | Illustrative scenario

A pharmaceutical manufacturer introduces a minor recipe modification.

A small quantity of ethanol is added to improve powder granulation.

The mixer remains unchanged.

The motor is correctly Ex-certified.

The dust extraction system is operational.

Production considers the modification insignificant.

But has the explosion hazard changed?

The original assessment was based on dry-powder characteristics.

The modified mixture may have different ignition sensitivity, vapour-generation behaviour and explosion severity.

The existing hazardous-area classification, ignition-source assessment and explosion-protection arrangements may no longer represent the actual process.

Required management decision

Before releasing the modified recipe, Process Engineering and Ex Engineering must establish whether representative material testing or reassessment is required.

Owner: Process Engineering Manager, supported by the Ex Responsible Person.

Required evidence: Approved recipe modification, relevant explosion characteristics, updated risk assessment and documented production authorisation.

Measure: 100% of material changes affecting explosion-protection assumptions assessed before implementation.

▶️ TODAY'S Ex QUESTION

“When was the last time we verified that our material explosion data still represent the substances actually processed?”

Check one powder-handling installation.

Compare the original design data against today's material composition, moisture content, particle-size distribution, additives and operating conditions.

Then verify whether the existing explosion-prevention and protection measures remain suitable.

DAILY Ex | CLOSING THOUGHT

Today's developments connect three essential aspects of industrial safety.

The material determines the hazard.

The equipment establishes the engineered protection.

The operating conditions determine whether that protection remains effective.

A certified motor cannot compensate for an unassessed change in material properties.

A correctly installed dust collector cannot compensate for inadequate airflow or accumulation of combustible material.

And a gas-detection system cannot protect personnel if its alarms do not trigger appropriate action.

Operational Ex means maintaining the connection between process knowledge, engineering assumptions and operational decisions throughout the installation's lifetime.

For the Head of Operations, the objective remains unchanged:

Know what has changed. Understand its consequences. Verify the protection before continuing operation.
