TODAY'S FOCUS: A process can be operating normally until one unexpected release changes everything.

Fatal explosions in Cameroon and India, lessons from metal-powder manufacturing, IECEx repair competence and industrial technology developments lead today's edition.

For Heads of Operations: Preventing explosions requires control of containment, ignition sources, equipment condition and operating changes-not simply the presence of Ex-certified equipment.

🔴 1. CAMEROON | Four killed in iron-smelting plant explosion

CURRENT INCIDENT - 2 November 2026

An explosion at an iron-smelting facility in Douala killed at least four people and severely burned 16 others.

The blast occurred at facilities operated by Metafrique Steel in Bassa Industrial Park.

A fire followed the explosion and was subsequently extinguished.

The company referred to an external factor, but the initiating cause has not been established.

Why it matters operationally: Metal-processing explosions may involve molten-metal interactions, pressure systems, combustible materials or other mechanisms.

An explosion at a metal-processing plant is not automatically a combustible-dust explosion.

Management action: Review molten-metal handling, cooling systems, process isolation, equipment condition and emergency arrangements.

🔴 2. INDIA | Gas-pipeline explosion kills one, injures six

CURRENT INCIDENT - 2 November 2026

A reported piped-natural-gas leak triggered an explosion in Muzaffarpur.

One person died and at least six others were injured.

The explosion severely damaged a four-storey building and shattered windows in the surrounding area.

The circumstances leading to the reported leak remain unclear.

Why it matters operationally: A gas release can create an explosive atmosphere outside an originally classified hazardous area.

Management action: Verify gas-pipeline integrity, isolation arrangements, leak detection and contractor controls.

⚙️ 3. TÜRKİYE | Metal-powder production explosion injures seven

RECENT INCIDENT - 1 October 2026

An explosion at a metal-powder manufacturing facility in Torbalı injured seven workers, three seriously.

The facility reportedly processed aluminium, zinc and copper powders.

The explosion involved a large processing vessel, but the initiating mechanism has not been confirmed.

Why it matters operationally: Metal-powder production can combine combustible-dust hazards with thermal, mechanical and pressure-related risks.

Engineering lesson: Assess the complete process, including vessels, powder transfer, collection systems, ignition sources and abnormal operating conditions.

📘 4. IECEx | Certified repair facilities and the limits of certification

CERTIFICATION PRACTICE - Current guidance, not breaking news

The IECEx Certified Service Facilities Scheme addresses inspection, maintenance, repair, overhaul and reclamation activities within defined certification scopes.

An important distinction appears in the official IECEx guidance:

A facility certified for inspection and maintenance does not thereby become authorised to issue IECEx equipment or installation certificates.

Its inspection reports can, however, support applicable regulatory or contractual requirements.

Why it matters operationally: An inspection certificate, service-facility certificate and equipment certificate demonstrate different things.

Management action: Verify the contractor's actual certification scope and the documentation required for return to service.

⚖️ 5. ATEX / IEC | Equipment assemblies require system-level assessment

STANDARDS WATCH - Continuing development

IEC 60079-46 addresses equipment assemblies used in explosive atmospheres.

The important engineering question is whether combining individually certified components introduces additional ignition hazards or changes the conditions under which those components remain safe.

This is particularly relevant to packaged pumps, compressors, conveyors, filtration systems and mobile machinery.

Why it matters operationally: An Ex-certified motor, instrument or enclosure does not automatically establish the explosion safety of the complete machine.

Engineering lesson: Assess mechanical ignition sources, electrical interfaces, protective functions and the complete operating arrangement.

🔬 6. USA | A fatal chemical release during plant decommissioning

INVESTIGATION LEARNING - CSB update published 13 August 2026

The U.S. Chemical Safety and Hazard Investigation Board investigated a fatal hydrogen-sulfide release at a catalyst-recycling facility in West Virginia.

On 22 April 2026, a chemical reaction occurred during the transfer of materials into a wastewater receiving tank.

Two employees died and four others were seriously injured.

The incident occurred in connection with decommissioning activities.

Why it matters operationally: Shutdown and decommissioning can introduce chemical combinations and operating conditions not encountered during normal production.

Historical lesson: Before transferring residual chemicals, establish their composition, compatibility and potential reaction hazards.

⚙️ 7. TECHNOLOGY | Ex inspection and maintenance need more than digital records

GOOD ENGINEERING PRACTICE

Digital inspection platforms can improve equipment identification, defect tracking and maintenance planning.

However, IECEx service-facility guidance makes clear that inspection and maintenance activities remain subject to technical requirements and defined competence.

Why it matters operationally: Recording an inspection does not establish that the inspection was technically adequate.

Management action: Connect digital inspection records to equipment identification, applicable protection concepts, competent personnel, defect classification and documented corrective actions.

📅 8. UAE | ADIPEC 2026 continues in Abu Dhabi

CURRENT EVENT - 2–5 November 2026

ADIPEC 2026 continues today in Abu Dhabi.

Its technical program covers energy operations, digitalisation, asset integrity, process safety and industrial technology.

Why it matters operationally: New inspection and automation technologies should demonstrate measurable improvements in reliability, maintenance effectiveness and personnel safety.

▶️ OPERATIONAL Ex USE CASE

The Ex vibrating motor - when more vibration means less bearing life

PRACTICAL | Illustrative scenario

A food-processing plant operates a vibrating conveyor handling combustible powder.

The conveyor uses Ex-certified vibrating motors.

Production wants to increase throughput.

Maintenance increases the vibration intensity by adjusting the motors' eccentric weights.

The modification improves material flow.

However, the higher vibration increases mechanical loading on the motor bearings and conveyor structure.

Over time, bearing deterioration may increase frictional heating.

The operational question: Does the original Ex certification cover the revised mechanical operating conditions?

Not necessarily.

The permitted adjustment range, mechanical loading, mounting arrangement, bearing temperature and operating limitations must be verified against the manufacturer's instructions and certification documentation.

The conveyor's non-electrical ignition hazards also require consideration.

Required decision: Engineering must determine whether the adjustment remains within the approved equipment configuration and whether the complete machine remains suitable for the actual dust environment.

Owner: Maintenance Manager, supported by Mechanical Engineering and the Ex Responsible Person.

Required evidence: Manufacturer's permitted settings, documented adjustment, assessment of mechanical ignition hazards, vibration and temperature measurements, and return-to-service authorisation.

Measure: 100% of vibration-setting changes assessed and documented before continued operation.

▶️ TODAY'S Ex QUESTION

“When production increases machinery output, who verifies that the explosion-protection assumptions remain valid?”

Consider rotating speed, vibration intensity, bearing loading, process temperature, dust concentration and extraction capacity.

A production improvement can unintentionally reduce the safety margin of an existing protective arrangement.

⚡ DAILY Ex | CLOSING THOUGHT

This week's industrial incidents demonstrate that explosions do not arise from one universal mechanism.

Some involve flammable-gas releases.

Others may involve process pressure, chemical reactions, combustible dust or mechanical failures.

The correct protective measures depend on understanding the actual hazard.

For Heads of Operations, this means looking beyond equipment labels and certificates.

Explosion protection must remain effective when production rates change, machinery wears, maintenance is performed and processes operate outside their original assumptions.

A certified machine is only one part of a protected process.

That is Operational Ex.
